Welcome to Three Rivers Hospice

Three Rivers Hospice, a division of Quality Life Services, has been serving patients in homes, nursing facilities, assisted and independent living facilities and hospitals across Western Pennsylvania since 1982. With over 35 years of hospice care experience, our team of industry-leading clinicians have one goal in mind – to maximize quality of life by providing compassionate care for people with a life-limiting illness. 

Hospice care is available to all individuals who are in need – this level of care is a Medicare benefit that is also covered by Medical Assistance (Medicaid). 

It is never too late or too soon to call – inquiries are welcome from anyone at any time. Our dedicated staff are available to answer inquiries, questions about coverage, and eligibility or simply to provide guidance.

Services

Bereavement Support

Our bereavement counselors provide grief and loss counseling, support groups, workshops and memorial services. Our bereavement support is provided while an individual is receiving hospice services and does not end with the loss of a loved one. We provide unlimited bereavement services to all family members as long as it is needed. 

Medical Equipment & Supplies

Medical equipment, medications and supplies, related to the patient’s life-limiting diagnosis, are covered 100% by Three Rivers Hospice. 

Don't Stop Dreamin'

A 501c3 non-profit organization that provides for the fulfillment of DREAMS of Three Rivers Hospice patient’s & Quality Life Services nursing and personal care home residents. Don’t Stop Dreamin’ recognizes that many individuals have unfulfilled DREAMS and desires. Every Three Rivers Hospice patient is eligible for a dream fulfillment. Hospice Medical Directors

Our team of medical directors provide guidance and support for our multi-discipline hospice team. They work closely with the patient’s Primary Care Physician to ensure seamless quality care. 

Registered Nurses

A registered nurse will coordinate and provide for care needs in addition, the RN is responsible for ongoing education and support of the hospice team members. In addition to regular visits, our nurses are available 24/7 for emergent visits. 

Medical Social Services

Based on specific needs, social workers are available to provide guidance about health care decisions, advance directives, legal matters and access to community resources.

Spiritual Care

Our full-time Chaplains provide spiritual and emotional support regardless of denomination. 

Primary Care Physician

The patient’s existing Primary Care Physician continues to lead and manage your plan of care. Our clinical team works hand-in-hand with the PCP as he/she continues to prescribe medications and monitor the patient’s needs. 

Hospice Aides

Under the supervision of the RNs,  hospice aides will provide personal care such as bathing, dressing and changing of bed linens.

Volunteers

Our volunteers are hand selected, trained, and compassionate community members. They provide a range of support services to patients and families such as patient companionship and respite for the patient’s caregivers.
